Pułk wywodzi swe początki od Ośrodka Organizacyjnego Broni Pancernej sformowanego na terenie Związku Radzieckiego w większości z jeńców wojennych. Po ewakuacji Armii Polskiej do Persji, a następnie przerzuconym do Egiptu i Palestyny 01.06.1942r. utworzono 1 Batalion Czołgów, którego dowódcą został mjr Stanisław Gliński. W dwa miesiące później 04.08.1942r. Batalion został przemianowany w 4 Batalion Czołgów i przerzucony do Iraku w rejon pól naftowych Khanakin. Dnia 01.07.1943r. 4 Batalion Czołgów został przemianowany w 4 Pułk Pancerny. W grudniu pułk ponownie został przerzucony do Egiptu i przygotowywał się do udziału w walkach na terenie Włoch. Na teren Włoch pułk przybył w kwietniu 1944 roku i w nocy z 11-12.05.1944r. wszedł do walki na Monte Cassino. 05.06.1944r. pułk został przemianowany w Pułk 4 Pancerny “Skorpion”. Ponadto pułk brał udział w walkach nad Adriatykiem w Apeninach i zdobyciu Bolonii. Po zakończeniu działań bojowych pułk pozostał na terenie Włoch w miejscowości Potenza
